Description
In 2005, the Child Fatality Prevention Act established the Child Fatality Prevention System (CFPS), a statewide, multidisciplinary, multi-agency effort to prevent child deaths. Child fatality reviews serve to understand child deaths in order to prevent the injury and death of other children in the future.

Because each local team will develop a unique structure, it is understood that this guidance may not be the best model for all teams. As such, teams are welcome to adapt from this model to accommodate the needs of their individual team.
