Description
In May 2012, Colorado submitted a proposal to the Center for Medicare and Medicaid Innovation (CMMI) within the Centers for Medicare and Medicaid Services (CMS) outlining its plan to integrate care for approximately 45,000 Coloradans who are fully eligible for Medicare and Medicaid and are not already participating in another recognized system that provides care coordination. The Department intends to expand upon its current Accountable Care Collaborative (ACC) managed fee-for-service program which is providing care for a number of Medicaid clients in Colorado. Using the ACC, the Department's goal is to improve care for Medicare-Medicaid enrollees in Colorado, improve health outcomes and client experience of care, and reduce unnecessary and duplicative services and resulting expenditures.
