Description
Colorado was awarded a five-year $22 million federal grant to implement the Colorado Choice Transitions (CCT) program, the Money Follows the Person (MFP) Demonstration. MFP is a federal grant that supports states' efforts to build and improve the infrastructure that supports home and community-based services (HCBS) for people of all ages with long-term care needs. The vision of the national initiative is to transform long-term care services and support from facility-based and provider-driven care, to person-centered and consumer-directed, community-based care.
